What we do
Perth Home GP is a privately billed, mobile GP practice serving the Perth metropolitan area. We offer daytime and after-hours home visits for patients who cannot access traditional GP clinics.
Services we provide:
- One-off home visits for patients with temporary barriers to clinic-based care.
- Ongoing, regular home GP care for patients with long-term barriers to clinic based care with home visits and telehealth
Our mission
Our mission is to bridge the gap in home-based GP care by providing comprehensive medical services to patients who struggle to access in-clinic care.
We collaborate with Home Care Providers, GPs, Allied Health Professionals, Nurses, and Specialists to deliver holistic and continuous care at home, ensuring vulnerable patients receive the consistent and integrated healthcare they need.
All our doctors are GPs with fellowship of RACGP or ACRRM.
Connecting home care providers with technology
Perth Home GP runs on Airclinix. Airclinix is a mobile practice management system developed by the founders of Perth Home GP, built to run a highly automated home care service from a mobile device. The platform is being developed as an ecosystem, allowing all home care providers to join with Perth Home GP and offer a seamless integrated care experience to patients at home.
Airclinix will soon be available for non-GP home care providers to run their mobile practices.
